For the Mobile Security Key, the serial number is assigned to you when you set up your mobile security key.

The serial number is given to you when you log into swtor. If you haven't added it before then you don't need to fill that out when you go to login.
If you have added one before you'll need to remove the existing one before you can add a new one. To remove the existing one just enter your Display Name, Password and then leave Security Key blank, click login. On the next screen click "Forgot your security Key" to then receive an email to remove it.
